Short answer
A live dental answering service costs roughly $130 to $915 a month for a small office taking 40 to 100 calls, based on published prices from AnswerOurPhone, Ruby and Smith.ai checked October 5, 2026. Leap, an AI receptionist, is $149 a month with the first 100 calls included, then $0.99 a call. A very quiet office can pay less on a small live plan.
How much does a dental answering service cost per month?
A live answering service for a small dental office costs about $130 to $915 a month at 40 to 100 calls, depending on the vendor and how it bills. Many services don't publish prices at all. The three below do, and we read each pricing page on October 5, 2026.
| Service | How it bills | Entry plan | Next plan up | Past the plan |
|---|---|---|---|---|
| AnswerOurPhone (live) | Per minute, in 1-second increments | $68.95 for 35 minutes | $129.95 for 90 minutes | $1.75 a minute ($1.70 on the 250-minute plan) |
| Ruby (live) | Per minute | $250 for 50 minutes | $395 for 100 minutes | Not listed on the pricing page |
| Smith.ai Human-first (live) | Per call | $300 for 30 calls | $810 for 90 calls | $11.50 a call on Starter, $10.50 on Basic |
| Leap (AI receptionist) | Per call | $149 for 100 calls | None | $0.99 a call |

How do per-minute and per-call billing add up?
Per-minute plans charge for the time an operator spends on your calls, so the bill depends on call length. Per-call plans charge the same for a 40-second call and a 6-minute one. To estimate either, take three numbers from your phone bill or carrier portal: calls a month, average call length, and how many of those calls come after you close.
- Per minute: calls a month times average minutes gives your minutes. Pick the plan, then add the overage rate for every minute past it.
- Per call: calls a month against the included calls, plus the per-call rate for the rest.
- Rounding: AnswerOurPhone bills in 1-second increments. Ask any other per-minute vendor whether it rounds each call up to the next minute, because that can add close to a minute per short call.
What does a small dental office actually pay?
At 100 calls a month, a small office would pay $322.45 to $915 on the live plans below and $149 on Leap. The table uses one assumption you should replace with your own: each call averages 2 minutes of operator time. That covers a greeting, taking the message and reading it back. These are worked examples, not data from any office.
| Service | 40 calls (80 min) | 100 calls (200 min) | 150 calls (300 min) |
|---|---|---|---|
| AnswerOurPhone | $129.95 | $322.45 | $435.45 |
| Ruby | $395 | $720 | $1,725 |
| Smith.ai Human-first | $415 | $915 | $1,440 |
| Leap | $149 | $149 | $198.50 |
Per call, that is about $3.22 on AnswerOurPhone, $7.20 on Ruby, $9.15 on Smith.ai and $1.49 on Leap at 100 calls. Call length moves the per-minute rows. At 3 minutes a call, 100 calls is 300 minutes, and AnswerOurPhone's figure rises to $435.45 while the per-call rows stay put.
The math flips for a quiet phone. AnswerOurPhone's 90-minute plan is $129.95, so an office using under about 100 minutes a month, roughly 50 calls at our assumption, pays less on that plan than on Leap.
How much does an AI receptionist cost?
Leap costs $149 a month, with the first 100 calls included, then $0.99 a call. It is month to month with no setup fee and no contract, and you can cancel from the dashboard; cancellation takes effect at the end of the billing period. Billing starts when Leap begins answering your real calls, so setup is free.
You keep your office number and forward calls to Leap. It builds its answers from your website and Google listing, and you review the facts it drafts before it goes live. After each call your team gets a summary by text or email, and each answered call is in the inbox with a recording and transcript. You can start here or read how Leap works for dental offices.
For a practice that handles patient information: we sign a Business Associate Addendum with your practice, and your calls run on a separate protected setup where patient information only goes to vendors covered for HIPAA. Ask us to turn it on. There is no separate price for it.
When is a live answering service still worth paying for?
A live service is worth the higher bill when your plan for night emergencies depends on a person, or when your phone is quiet enough that a small plan costs less. These are the cases we'd point to.
- Your night emergency protocol runs through a person. If an operator pages the on-call dentist and stays on the line with a patient in pain, a live service is built for that. Leap gives callers the emergency contact you approve, takes the details, marks the request urgent and texts your team. If a caller describes something life-threatening, it tells them to call emergency services. Decide whether that meets your protocol before you switch.
- Your phone is quiet. Under about 100 minutes of calls a month, AnswerOurPhone's small plans cost less than Leap.
- You need bookings written into your practice software. Leap does not connect to Dentrix, Open Dental or other practice management systems. By default it takes the appointment request and your team confirms the time. It can text callers your online booking link, and it can book directly into a connected Google Calendar.
- Your patients ask for a person. If patients already tell your front desk they dislike automated phone systems, weigh that against the savings.
Can a dental office use both?
Yes. One split is to let the front desk answer during the day, send after-hours calls to an AI receptionist, and keep a live service only for emergency paging. In Leap's forwarding setup you choose between forwarding every call or having your phone ring first, with Leap answering the ones you miss. Answering only after hours uses your phone system's own scheduling or a forwarding code you dial at close; this guide covers common office phones, and this post walks through an after-hours call.
What should you ask a vendor before you sign?
- Do you bill by the second, by the minute rounded up, or by the call?
- Which calls are free: spam, wrong numbers, hang-ups before anyone speaks?
- Will you sign a Business Associate Addendum?
- How does a message reach us, and how fast?
- Is it month to month, and is there a setup fee or a forwarding-number fee?
Smith.ai's pricing page says that as long as the lines you forward pass Caller ID, you won't be charged for spam or unsolicited sales calls. Get the same answer in writing from anyone you're considering. On the Business Associate Addendum: the HIPAA Privacy Rule lets a practice share patient information with a business associate only with written assurances (45 CFR 164.502(e)). A service that takes patient messages for you falls in that category, whether a person or software answers.

Frequently asked questions
How much does a dental answering service cost per month?
On public pricing pages checked October 5, 2026, live plans start at $68.95 for 35 minutes (AnswerOurPhone), $250 for 50 minutes (Ruby) and $300 for 30 calls (Smith.ai). At 100 calls of about 2 minutes each, the same three come to roughly $322 to $915 a month.
How much does an AI receptionist cost?
Leap is $149 a month with the first 100 calls included, then $0.99 a call. It is month to month with no setup fee. Other AI receptionists price differently, so compare what each includes.
Is per-minute or per-call billing cheaper for a dental office?
